Rigid preshaped mesh

The position of a rigid mesh on the floor of the inguinal canal is always flat, without wrinkles or dead spaces.

Soft preshaped mesh

A soft mesh may wrinkle, form dead spaces and cause seromas.

Closed anatomical space

A sutureless mesh cannot move or migrate inside an anatomical space closed by fascial and aponeurotic tissue.

 

Sutures and Tension

A mesh sutured to any tissue may produce maldistribution of tension and dead spaces. A sutureless mesh, especially if it is rigid, is always tension-free and does not produce dead spaces.

Triple Layer

When the external oblique aponeurosis is closed over the chord,the medial aspect of the inguinal canal is weak, because there is only mesh and transveralis fascia. When the aponeurosis is closed under the spermatic chord, there is medially a triple layer: Transersalis fascia, Mesh, and External oblique aponeurosis.

When the spermatic cord is in a sub fascial position, the mesh is in contact only with the transversalis fascia in the medial aspect of the floor of the inguinal canal. When the spermatic cord is in a supra fascial position, the mesh is in contact with the transversalis fascia and the aponeurosis of the external oblique muscle. A stronger triple layer is formed in a critical area, where recurrence are more frequent.
